Required documents

Proof of identity (photo ID, social security card, birth certificate). Proof of residency (utility bill). Proof of income (two pay stubs, social security or worker's comp income, unemployment).

Hours

Mon-Thu 8 am-11:30 am, 1 pm-4 pm. Fri 8 am-11:30 am, 1 pm-3 pm. Walk-ins accepted starting at 7:30 am. Summer Dental Clinics (extractions only), registration begins at 4:30 pm on the following dates: July 15th and 29th, August 5th and 19th, 2024.
