Case management services for HIV+ individuals include, but are not limited to, assistance with budgeting, nutrition, completion of OHDAP applications to receive assistance with medications, co-payments, and insurance premiums, and information and referrals to agencies in the community who can help HIV infected clients and their families reach stabilization and self-sufficiency. Monthly support group for HIV+ individuals with a focus on reducing stigma, socialization, and disclosure. Behavioral health services open to ALL (not just those HIV+)

Application process

Call for information. If someone is too ill to call on their own behalf, they may request that someone else call for them. However, services are only available to individuals who desire services for themselves.

Eligibility

Case management services and support group is open to individuals living with HIV. Behavioral healthcare and counseling services is open to ALL.
