Call and schedule a new patient exam. Walk-ins welcome, but appointments preferred.For Patient Transportation Service, call 419-522-6191.

Fees

All insurances accepted including Medicare and Medicaid. Sliding fee is available for qualifying patients. Sliding fee means the bill may be reduced, not free. Please be prepared to make the minimum payment required at every visit.
