There are no restrictions for this service. Anyone may pick up a birth or death record.
Required documents
Valid identification may be requested.
Hours
Monday - Friday: 8:00am to 4:00pm. Curbside/walk up-window support may be limited between Noon and 2:00pm, but indoor service windows are open during this time.

Call the Office of Vital Statistics or visit the website for more information. Order by walk-in/kiosk same day service, curbside/walk-up window, internet, or phone.
Walk-in/Same Day Service/Kiosk Service:
Visit during regular Vital Records office hours of operation.
Internet Orders:
Go to website at www.scph.org/birth-death-records. Orders are processed the next business day.
Phone Orders:
Call the Office of Vital Statistics at 330-812-3845. Orders are processed for same day pick up or mail delivery.
Fees
Walk-in/Same Day Service:
Fixed fee: $22 each. Accepts cash, personal check, money order, credit, or debit cards (additional fees apply for credit/debit card).
Internet Orders:
Fixed fee: $22 each with additional fees for shipping and credit/debit cards. Current shipping options include United States Postal Service (USPS) Certified Mail (2-5 business days) and USPS Priority Mail (1-3 business days). Delivery fees vary by address destination.
Phone Orders:
Fixed fee: $10 fee for phone service. Each certificate costs $22 with additional fees for a credit/debit card. Accepts credit or debit cards.
