Provides services and supports such as respite, education, training and/or supplies for families that have an individual with a disability residing at home and residential services and supports such as assistance with budgeting, self-care and community participation for individuals with a disability that reside in the community.

Application process

All individuals determined eligible for service through the Ohio Eligibility Determination Instrument (OEDI) or Children's Ohio Eligibility Determination Instrument (COEDI). If eligible, proceed with enrollment and assessment for services. If not eligible, refer to appropriate resources.

Eligibility

Must be a resident of Madison County and have a physical or mental impairment other than mental illness that manifested prior to the age of 22. Must meet eligibility requirements of the County Board (exhibit at least three substantial functional limitations).
