Eligibility
Serves Veterans in Lorain County. All bills must be dated within the past 12 months of the application and have not gone to collections. Minimum of $100 in expenses shown to process application.
NOTE: For dental services, must first apply through this program and see an approved dentist to qualify.
Required documents
Completed application including:
- DD Form 214 or other discharge documentation.
- Current state ID for Veteran and all household members.
- Documentation of all household income and expenses.
- Itemized medical bills or statements.
- Other documents will be requested as needed.
