Provides educational programming to help seniors manage their type 2 diabetes, arthritis, hypertension, lung disease, and other illnesses and conditions. The goal is to slow the progression ofillness as long as possible without the use of more intensive health care services. Teaches relaxation techniques, healthy eating, managing sleep and fatigue, managing medications and monitoring blood sugar, exercise, and communicating effectively with healthcare providers.
