Application process

Phone to make a report during service hours. Reports may also be submitted by Internet, fax, or mail. Mailed reports should be sent to the attention of "OIG Hotline Operations." Reports need to include type of complaint and the department or program affected by claims. Include the name and address of the person, business, or department the allegation is against and a brief summary of the allegation. May be submitted anonymously, but helpful to include the name, street address, city, county, state, zip code, and e-mail address of the person filing the allegation. Information will be reviewed promptly by a professional staff member, but agency is unable to reply to submissions.
