Eligibility

Open to Lakewood residents age 60 or older. Availability of medical appointment rides for those in wheelchairs and scooters is limited as the agency has only one accessible vehicle. Also serves adults age 18-59 with a permanent physical or developmental disability.

Application process

Phone for information on application M-F: 8:00am-4:30pm. Medical Transportation can be scheduled three weeks ahead of a medical appointment and no later than three business days prior to a medical appointment. Client will need to register with a city social worker before service can be provided.
