Eligibility

Open to residents of Ohio who are:

  • age 18-59 with a disability (as defined by Social Security), or
  • age 60 or older

Photo ID,Other documents (Individuals qualifying due to disability only: Medicare card, SSI or SSDI acceptance documentation, or document certifying permanent and total disability by any state department, Ohio public employee retirement system, or any federal retirement system)

Application process

People under age 60 who are eligible due to disability must apply in-person at a sign-up site (most public libraries and senior centers). People age 60 or older can apply in-person at a sign-up site or online.
