Houses birth and death records for the county, dating back to December 1908. If a person was born in Ohio a birth record is available from the vital statistics. If a person died in Perry County, the record is available from the vital statistics office. Only certified copies are available of birth and death records. There is a fee. Payment is due at time of request. There is a form to complete. Requests are fulfilled immediately. There is no waiting time. Birth certificates from other Ohio counties may also be purchased through the Perry County Health Department.
