Must be a dialysis or transplant patient and live within the service area. Income must be at or below 200 % of the federal poverty level for ongoing assistance. $150 of emergency assistance is available to any dialysis or transplant patient with no income qualification needed.

Provides assistance to people with end stage renal disease who are on dialysis or who are newly transplanted. This includes assistance with transportation, medications or nutrition supplements. Must income qualify for ongoing assistance. Emergency assistance in the amount of $150/year is available on an annual basis.
