Application process

There are two dental insurance providers who are contracted to provide these plans. Phone to contact one of the contracted dental insurance providers for more information. Information about the program is available on the website (with links to the websites of the contracted insurance providers).

Fees

Each participant will pay a fixed monthly premium (reduced) for coverage in addition to any co payments required by their plan.
