  • Serves Medicare Part B recipients that are residents of Ohio.
  • Certain income and resource (assets such as bank accounts, vehicles, or property) requirements must be met.
Required documents
Must complete ODM (Ohio Department of Medicaid) 07216 (Application for Health Coverage & Help Paying Costs) and provide supporting documentation.

Visit the website for details. Click on the "Cash, Food, & Medical Assistance" tab; from the list click on "Medicaid"; and then from the lists/links click on "Medicare Premium Assistance Programs". To apply for one of the Medicare Premium Assistance Programs:
  • Call the call center to apply OR
  • Apply online application at https://benefits.ohio.gov OR
  • Print out a ODM 7216 Application for Health Coverage & Help Paying Costs and submit it via U.S. Mail, Fax (440-323-3422), or email ([email protected])
  • Submit application to Lorain County Job and Family Services (LCJFS) via mail, fax, email ([email protected]).
