Eligibility
  • Serves individuals who need non-emergency medical care far from home (usually between 100-1000 miles from home) and have a financial or medical need for assistance.
  • Individuals must be able to sit upright in a standard aircraft seat, must be able to board the aircraft with minimal assistance, and must provide transportation to and from the airport. Cannot transport wheelchairs, wheelchairs must be shipped or rented at destination.
  • Adults may bring one (1) companion and children may bring up to two (2) companions.
  • Must be able to obtain a physican's medical release for the flight.
Required documents
Completed application. Must provide doctor's name and fax number for physician's release form.
