Fee
- Accepts Medicaid/Medicaid Managed Care, Medicare Managed Care, many commercial insurance plans, and private pay. - Minimum fee for dental clients who qualify for a 100% discount based on Federal Poverty Guidelines. - Sliding scale based on income, and other financial assistance, available for adults. - Sliding scale available for youths age 17 years and under that have a letter from Job and Family Services denying health care coverage. - Call the office and speak with the Billing Department for details.
Eligibility
- Serves residents of all ages residing in Lorain County and surrounding communities. - Will not serve individuals with a particular condition/illness who has already been diagnosed and seen by another dentist for that condition/illness.
