Ages 13 and older. Populations served include adolescents (ages 13-17), adults (ages 18-54) those with a dual diagnosis of mental health and substance use disorder, mood disorders, thought disorders, and geriatric psychiatric issues for those aged 55 and older.

Proof of Insurance if applicable. Proof of household gross income for past 12 months and proof of family size if sliding scale is desired.
