Provides an integrated program of home care services designed to address the specific home care needs of those needing nursing and/or daily living services. Two types of benefits packages are available: Home Health Services (formerly Core Services), and Waiver Benefits. Home Health Services include nursing, daily living, and skilled therapy; Waiver services, for those whose medical condition and/or functional status would qualify them for Medicaid coverage in a nursing home or hospital, can also include home delivered meals, emergency response systems, home modifications, adult day health services, out-of-home respite, social work and care coordination.
