Fees
- Accepts Ohio Medicaid, Medicare and some private insurances. - Co-payments will be collected at the time of the office visit. If need to make other arrangements regarding co-payments, call the office prior to appointment. - Patients may make payments with a personal check, credit card, debit card or cash. - Offers sliding fee scale based on income and family size. - No patient shall be denied service due to inability to pay.
