Application process

Call the office or visit the website for more information about the school-based program or to request service. Students are referred to the program through their schools.

Fee

Accepts most types of Medicaid (including CareSource, Buckeye and United Healthcare), Medicare Part B and eligible commercial insurance programs. Offers a special fee schedule for non-insured clients. The agency will verify and review coverage/benefits during the intake session.

Eligibility

Serves children and youth ages 3 to 18 in participating schools. The agency retains the right to determine from among applicants for services individuals the agency can serve appropriately, within the limits of their resources and mission.

Languages

Spanish and American Sign Language. Utilizes International Institute for other language interpretation.
