Self-directed home care support program. Eligible veterans are approved for annual budget that can be used to hire/purchase services to help avoid nursing home placement. Types of services provided include: * Personal care tasks * Meals or other home delivery services * Respite care * Care coordination * Goods and services to improve home safety * Case management provided by the Area Agency on Aging * Provided through collaboration with the Louise Stokes Cleveland VA Medical Center.
