Staff will discuss needed documents at time of intake.

Service is usually by referral from the county board. Those wanting information may phone M-F: 8:00am-5:00pm to schedule a face-to-face intake appointment or to inquire about the status of waiting lists.

Fees are generally funded via the Medicaid waiver program.

Operates residences for individuals with intellectual disabilities, where staff assist in development and practice of individual skills including meal preparation, banking, grocery shopping, travelling in the community, planning outings and participation in social activities. Group and individual residences are offered.
