Geriatric Case Management | Catholic Charities Community Services Of Medina County
Provides case management services for older adults including assessments and evaluations, advocacy, and referral services for health concerns, in-home safety, financial issues and ability of individual to take care of daily housework/activities. Assesses mental health concerns and provides in-home counseling.
