Those who are under age 60 must have a referral from a Social Worker verifying a physical disability that is severe enough to render the individual homebound. Phone to arrange for service Tu-Sa: 10:00am-2:00pm; if no answer, leave a message and the call will returned within 1-3 business days.

Income must be at 200% or less of the Federal Poverty Guidelines. In the City of Cleveland, ONLY serves the Old Brooklyn Neighborhood (Zip codes 44109 and 44144). Also serves the cities of Parma, Parma Hts., Brook Park, Middleburg Heights, Brooklyn, and parts of North Royalton and Berea. Serves individuals age 60 and older who meet income guidelines. No age restrictions for those who are homebound (having limited mobility to the point where they can't leave the home due to physical conditions). May receive service once per month.
