If you are a new patient, before picking up medication, call (please leave a message if no answer) to obtain eligibility information. You will be asked to fill out an Eligibility Attestation form prior to receiving your medications through the pharmacy.
Fees
$10 monthly donation is suggested. This is NOT REQUIRED to obtain services.
