Eligibility

Serves people of all ages who are homebound, disabled, chronically or temporarily ill. Must be a resident of Highland Heights, Lyndhurst, South Euclid, Mayfield Hts., Mayfield Village or Richmond Hts.

Application process

Intake begins with an in-home assessment to determine eligibility for the program. Call to schedule M-F: 9:00am-1:00pm. Upon completion of intake evaluation, eligible clients are scheduled for delivery of meals.

Fees

Fixed fee. There is a deposit required prior to the beginning of service - the deposit is credited as payment toward the last month of service.

Description

Offers a choice of a hot meal plan (meat, vegetable, starch, roll and butter, dessert, and milk), or combination hot meal and lighter meal plan. Combination plan includes the hot meal and a sandwich, fruit or dessert and milk. Offers a mild diabetic, low sodium diet. Meals are prepared under professional supervision by hospital staff.
