Application process
Call for more information. *Individual Service Plan: fill out application packet, meet program coordinator, tour facility. Executive Director makes final decision regarding who is accepted into programs.
Fee
Determined By Agency: Some programs require payment. Payment accepted through Ohio Dept. MRDD Waiver, United Way, & Private pay.
Eligibility
18 years or older, have a developmental disability that is: not solely diagnosed as mental illness, occurred before age 22, will continue indefinitely, and results in limitations in (3) self-care, language, learning, mobility, self-direction, independent living, and/or self-sufficiency.
