Muskingum County households with a member who is 60 years or older, or anyone with a qualifying medical condition. Income guidelines apply--call for details.

Under 60 years of age needs a prescription from doctor. Forms for your doctor are available. Over 60 years does not need a prescription from doctor. Proof of income for past 30 days (pay stubs, letters from Job and Family Services, Social Security, etc.) must be presented at the interview. Other documents include: Social Security cards for all members of the household, proof or disability, both bills for main heating and electric utility source, proof of address, photo ID (of person signing application), and document of co-payment if required. *A lease or letter from the landlord is required for all renters.*
