Persons who have been referred by a Physician, generally those with a disability or condition such as dementia. Physician must write a prescription for the evaluation including a diagnosis. Must be at least 15 1/2 years old and have a driver's learning permit or a driver's license. Must have a valid driver's license for the road test.
Doctor's prescription (including diagnosis), Driver's license or driving permit, insurance card.
By appointment M-F: 8:00am-4:00pm and Sa: 8:00am-12:00noon.

Provides an assessment of ability to drive for those persons whose driving ability may be affected by a disability or medical condition. Evaluates motor and cognitive skills by use of a driving simulator and a road test.
