Adults 60 or older, or 59 or younger with a disability.
May need the following information/documentation (dependent on the program): date of birth and Social Security Number for self and all those in the household Veteran status for self and spouse length of time in current residence a list of the prescription medications currently taking including dosages current check stubs (if employed) and income from all other sources for self, spouse and others in the household rent or mortgage payment amount current utility bills current cell phone bills. Will be advised if other documents/information are needed.
By appointment M-F: 8:00am-4:30pm. Appointments last approximately one hour.

Phone for a short phone screening and an appointment for on site or phone service. Those with disabilities who are under 18 must have a parent or guardian call for them.

Fixed fee; if unable to pay agency will provide referrals to agencies/programs that may help pay for the service.

Provides disability benefits assistance by offering information to people with disabilities relating to federal and state benefits. Provides assistance with resources such as food stamps, housing, child care, Medicaid, transportation, as well as vocational rehabilitation services. Assists with applications and appeals for Social Security disability benefits.
