Provides public insurance to cover medical services that may include doctor visits, some prescription drugs, hospital care, home care, long-term care, medical equipment, transportation, mental health, vision, dental, substance abuse services and other services. Application process

- The Department of Job and Family Services receives a monthly list of those aging out of foster care, documents the clients' unique status, and opens the Medicaid case. - If someone meets criteria, is not currently receiving coverage, and wishes to apply, they should be clear about their history as a foster child when applying. This should prompt the eligibility worker to explore the coverage. - Clients will need to reverify yearly after age 19. - Clients may call the Ohio Benefits Phone Line Contact Center Monday through Friday from 8:00am to 4:00pm to begin the application process, renew, and/or update their case information. May also apply, renew, and/or update case information through website anytime (client must have an email address to do this). - If preferred, may also pick up and drop off paper applications and submit documents on-site Monday through Friday from 8:00am to 4:30pm. - NOTE: MCP-eligible clients must select and enroll in an MCP, or will be auto-enrolled randomly. Plan can be changed with reason, or during open enrollment. - In most cases, any interviews can be done through the Contact Center. - If applying by phone, the interview may be done during the phone call. - If applying via website or paper, the agency will schedule an over the phone interview (client will be given a day and time frame to call in). - If preferred, may also pick up and drop off paper applications and submit documents on-site Monday through Friday from 8:00am to 4:30pm. - NOTE: MCP-eligible clients must select and enroll in an MCP, or will be auto-enrolled randomly. Plan can be changed with reason, or during open enrollment.

Fee

No fee to apply. Once receiving coverage, individual MCPs may charge co-payments for some services.

Eligibility

- Serves those age 18 through 26 who have aged out of foster care and who reside in Portage County. - Those who age out of foster care will be covered through age 26 and are eligible to start coverage at any point before age 26. There are no income or asset requirements for this group. - Those who aged out of foster care in other states, and who meet these guidelines are eligible for coverage as part of this group. - Those who left foster care before aging out are not eligible for coverage as part of this group, but may be eligible as a different eligibility group.
