Hospice Care At Hospitals And Extended Care/nursing Facilities | Hospice Of The Western Reserve
Provides comfort, care, and emotional support for persons with life-limiting illness (life-threatening or terminally ill) and their families. Services include home health assistance, bereavement counseling, legal assistance, social work, art and music therapy, and spiritual care. Offers specialized AIDS and pediatric care teams. Respite care is provided for client's families.

Application process
Phone 24 hours a day, 7 days a week for enrollment information and to schedule an in-home assessment. An assessment will be done to see if the person is appropriate for this program.
Fee
Fixed fee but no one is denied service because of an inability to pay. Third party payment accepted: Medicaid; Medicare; most private insurance. A Patient Financial Advocate is available to evaluate financial assistance options.
Eligibility
Other eligibility may apply, but generally: Adults 18 and older who are terminally ill (life expectancy 6 months or less); Youth under 18 who are terminally ill (generally have a life expectancy of 1 year or less) or have a life-limiting diagnosis and are in a state of decline. Must reside in a private home, an extended care facility, hospital, nursing home, assisted living or developmentally disabled facilities in Ashtabula, Cuyahoga, Geuaga, Lake, Lorain, Medina, and Summit Counties, with outreach in Portage county.
