Residential Care - Developmental Disabilities | Blossom Hill

Provides total care services for those with developmental disabilities at their facilities (Center Ridge House, Haven Home, and Blossom Hill.) Services include individual plans, vocational training, speech, occupational and physical therapy, recreational activities, nutritional and psychological services and 24-hour nursing care.

Application process

Phone for information M-F: 9:00am-5:00pm. Referral must be made in writing by CBDD. Openings are rare, and waiting list is administered through CBDD. Out-of-county clients are accepted contingent on reimbursement of Cuyahoga County Board Developmental Disabilities. Transportation provided for residents to various activities and appointments.

Services generally paid for by County Board of Developmental Disabilities (CBDD) and/or Medicaid.

Eligibility

Medicaid eligible Ohio residents age 16 and older with a developmental disabilities. Center Ridge House serves non-mobile individuals with profound medical disabilities. Haven Home accommodates individuals with physical disabilities who have high medical needs. Blossom Hill serves individuals with behavioral needs. Parma House serves moderate to severe physical and developmental disabilities.
