Eligibility
Individuals with disabilities in the process of vacating nursing home facilities or who are in jeopardy of losing their independence through forced placement within nursing home facilities.
Required documents
Call for information and instructions. Intake packet must be submitted.
Hours
Office hours: Mon-Fri 8:30 am-4:30 pm. Appointments available and necessary during any federally and state mandated regulations. Scheduling intake appointment recommended.
Languages
We are not multi-lingual, but if proper notification we will work to meet every accommodation possible to service our consumers.
