- Serves Veterans residing in Lorain County who hold an Armed Forces discharge record known as a DD Form 2104 or DD214. The discharge record must state the words: Honorable Discharge. - An original or certified copy of the DD214 must be presented to be recorded.
A valid original or certified DD214 (DD Form 2014) displaying an honorable discharge status and two (2) of the following forms of identification (one (1) of which must contain a photo): - A Social Security card. - An original or certified birth certificate. - An identification card issued by the United States Department of Veterans Affairs. - A United States military identification card. - A license or permit to carry a concealed weapon issued by the state of Ohio or any other state. - A motor vehicle operator's license issued by the of state of Ohio or any other state that bears a photograph of the individual identified. - An identification card issued by the state of Ohio or any other state that bears a photograph of the individual identified. - A valid Passport that bears a photograph of the individual to whom the passport was issued. - A United States armed forces discharge record. For Renewal: - Do not need to present military discharge papers. - Must present current Lorain County Veteran identification card or a proper form of identification displaying a photograph.
Must be renewed every 10 years.
