If the company is a corporation, LLC or LP, list the Ohio Corporate Charter number, Ohio Foreign License number, or Articles of Organization number issued by Ohio's Secretary of State. Also needed are the names and addresses of the corporate officers.
Fixed fee. The annual fee for a vendor license (ST-1) is payable by check or money order.
