Applications: In agreement with the Social Security Administration (SSA), the Division of Disability Determination (DDD) determines medical eligibility for Ohioans who apply for Social Security Disability Insurance (SSDI) or Supplemental Security Income (SSI) benefits through the Social Security Administration (SSA). This can be done in one of three ways: - Find information online at www.ssa.gov. - By telephone at 800-772-1213 . - In person at a local SSA office. Appeals/Complaints: Social Security Disability Insurance (SSDI) and Supplemental Security Income (SSI) claims. If an individual is determined ineligible for Social Security disability benefits or Supplemental Security Income, he or she can request an appeal. An appeal can be initiated by contacting SSA. Per SSA guidelines, the appeal must be submitted within 60 days of the written notice of the decision.

Opportunities For Ohioans With Disabilities

This state agency provides services leading to employment for people with disabilities, beginning at age 14, including visual impairments. Services may include evaluation, counseling, personal adjustment training, placement and follow up. Other services may include independent living services available for visually impaired individuals who are 55 years of age or older.
