Community Advocacy Program (cap): Reentry Clinic | Metrohealth Medical Center
Provides an intake point for members of the reentry community who need health care, especially for chronic conditions, including mental health disorders. Each patient works with a financial counselor on qualification for free or reduced cost care and prescriptions. New patients see a doctor, get any needed prescriptions written, then choose a convenient MetroHealth clinic and a primary care doctor for ongoing care. Prescriptions will NOT be written until the patient is seen for an appointment (or several appointments, in the case of mental health assessment). A legal assistant on-site assesses legal needs, then assists with applications (e.g. Medicaid, Social Security), appeals of benefit denials, and referrals to Legal Aid services, as well as education regarding legal rights.

Call M-F: 8:00am-5:00pm and ask for ReEntry Clinic, to make an appointment. Staff also visits prisons, and (with client permission) can sometimes make arrangements and obtain previous medical records before client is released.

Each patient is rated for the MetroHealth sliding scale.

Open to residents of Cuyahoga County who have previously been incarcerated.
