Eligibility
Adults 18 years or older with 1 year documented history of opiate addiction.
Required documents
Drivers license or State ID, proof of 1 year history of opiate addiction.
Hours
Primary Care (Entrance A): Monday-Thur, 6am-4pm
Fri 7am-4pm
Chiropractic, Acupuncture & Occupational Health Services (Entrance A): 8am-4pm
Outpatient Counseling (Entrance B): 8am-4pm
Dosing (Entrance C): Mon-Fri, 5:30am-1:00pm
weekends and holidays, 5:30am-11:00 am. Closed on Sundays.
MAT Counseling (Entrance C): Mon-Fri 5:30am-5:30pm
