Serves income-eligible pregnant women and families with children from birth to age 3 residing in Summit County. Parents must be working and/or attending school (minimum of 35 hours per week and/or full-time student status).
PARENT'S INFORMATION Proof of Income: - If Employed: Copy of previous year's W-2 or written statement from employer. - If Self-Employed: Copy of 1099 form. - If Unemployed: Copy of OWF, SSI, Social Security, Unemployment, Child Support, and so forth. CHILD'S INFORMATION: - Birth Certificate/Birth Record. - Up-to-Date Shot Record. - Custody/Guardianship papers, if applicable. - Names of two (2) emergency contacts. Must have name, address, and phone number. - Doctor's and Dentist's name, address, and phone number. - Insurance Card or Health Card.

Offers two (2) program options: - Center-based option offers 5-days a week (full-day or half-day) in a classroom setting. - Home-based option offered once a week for an hour and a half in a home setting.
