-Your children’s birth certificates
-Social security cards and/or birth letter from hospital
-A completed Noncustodial Parent Questionnaire
-A completed JFS 07076 – Application for Child Support Services Non-Public Assistance Applicant/Recipient

Friday: 7:30 am to 11:30 pm.
