Must be a woman who is pregnant or - Lactating with a baby up to one year of age, or - Be postpartum for up to six months after the termination of a pregnancy by birth, miscarriage, or abortion - Those younger than 18 do not need parental/guardian permission - Children are eligible up to fifth birthday - Eligible individuals must live in Lucas County - Must be at nutritional risk as determined by an agency professional using the Federal Nutritional Risk Priority System and - Must be at 185% of the Federal Poverty Guidelines - Number in household includes unborn children (Example: A pregnant, single woman is a household of 2 - Definition of income does not include household assets - Individuals receiving Medicaid, Ohio Works First or Food Stamps automatically meet income eligibility

- ID that shows name and birth date for those in the household who are applying for benefits (could even be a crib card from the hospital). - Proof of pregnancy from doctor or clinic, unless the pregnancy is clearly visible - 3 pay stubs, SSI statements, proof of Medicaid, Ohio Works First, Food Stamps or other means of verifying income, a written statement from child's father saying he is paying child support - Proof of child's current immunizations - Proof of residency (including utility, telephone, or credit card statement)
