Eligibility
Individuals under 60 years old with a disability who reside in Ashtabula, Columbiana, Mahoning, and Trumbull Counties who require assistance with certain activities of daily living. Eligibility for specific services is determined by a care manager during an in-home assessment.

Offers assessment and case management for individuals with disabilities to assist them with remaining in their home as long as possible. Through case management services, participants have access to a variety of in-home services, such as assistance with chores, home modification/barrier removal, home delivered meals, accompaniment to and from medical appointments, personal care assistance, etc.
