Open to Duke Energy customers in threat of disconnection who have a qualifying medical condition.

Other documents (Written verification from qualifying medical professional stating that termination of utility service will aggravate the medical condition of the resident. )

Ohio customers may complete the Medical Certificate Request Form online

Kentucky customers should contact Duke Energy Customer Service by phone to request a Medical Certificate Form be faxed to their medical provider.

Provides a 30 day extension to prevent utility disconnection with verification of qualifying medical condition from a medical provider. Ohio customers are permitted a maximum of three medical extensions within a rolling 12-month period and Kentucky customers are permitted medical certificates as long as customer has not defaulted on a previous medical certificate payment plan.
