Offers a reentry program for individuals at risk of homelessness with severe and persistent mental illnesses or HIV diagnosis exiting the prison system. Supportive services include case management, behavioral healthcare, employment, reentry support, and rental subsidies for housing.

Provides community-based behavioral and physical healthcare services in Summit County as well as housing support, homeless outreach, and supported employment. Serves as a safety net for those who suffer from illnesses such as schizophrenia, bipolar disorder, major depression, and personality disorders. Services include psychiatric and medical programs, rehabilitation, advocacy, and support for recovery. Agency is a Certified Community Behavioral Health Clinic (CCBHC).
