- Clients may apply or update their information by calling the Contact Center or via the website. - NOTE: Those who are waiting for disability determination by Social Security can receive their disability determination for Medicaid through the county. This can take weeks to months.

Eligibility

Individuals of any age who need long term care. Must live in Medina County and be a U.S. citizen. Financial eligibility is an income at 300% Federal Poverty Level (FPL) and assets up to $2,000 for an individual, $3,000 for a couple, those whose income is higher must create a Qualified Income Trust (QIT).
