- Age 18 or older. - Reside in Summit, Cuyahoga, or Medina County. - Participating in substance use disorder treatment. - Have been diagnosed or participated in SUD treatment in the last 10 years.

- Current driver's license or state ID. - Birth certificate. - Social Security card. - Proof of SUD treatment. - High School diploma, GED, or transcripts (if applicable). - Proof of residency.
