COMMUNITY SUPPORT TEAMING | LICKING COUNTY CHILDREN AND FAMILIES FIRST COUNCIL
Community Support teaming is available to all families with youth ages 0-21 who have multi-system needs, are at risk of out-of-home placement, or are returning from out-of-home placement. High Fidelity Wraparound Team Facilitators will assist the family in building a team, coordinate services with providers working with the family and provide linkage to community resources in an effort to ensure sufficient support is in place for the child and their family. Community Support Teaming is committed to strengthening families and improving the local social and human service delivery system by sharing resources and developing and coordinating services and resources for children, youth and their families.

The Council Office hours are 8am-4:30pm Monday through Friday. Council meetings are held the first Wednesday of August, October, December, February, April, and June, beginning at 1:30pm at the Licking County Health Department. Anyone can make a referral. Call or email the Multi-system Services Supervisor for information.

Eligibility
Families with child ages Birth-21 who have multi-system needs, are at risk of out-of-home placement, or returning home from out-of-home placement. Families with private insurance or no insurance can be served. OhioRISE eligible children who are Tier1 or who do not wish to receive the OhioRISE Care coordination services are also eligible.
