Provides centralized access to services and opportunities for individuals with developmental disabilities age three and older. Services include but are not limited to: socialization opportunities, workforce development, case management, personal care, behavior support, and respite care.

<p>New patients may schedule an appointment in advance by phone or walk-in for their initial visit. Ongoing services are provided by appointment.</p>

Fees are paid by Medicaid.
Eligibility
<p>Open to individuals with Intellectual/Developmental Disabilities (IDD) who meet Medicaid eligibility criteria. Psychological testing and adaptive behavior assessment must verify disability.
