Eligibility
Residents of Allen, Auglaize, or Mercer Counties, at or below 175% of Federal Poverty Guidelines, have a shut off notice, have been disconnected, have a household member age 60 or older, or have a physician documentation that cooling assistance is needed for a household member's health.
Required documents
Verification of past 30 days of income (12 months is preferred), Social Security Numbers of all in household, utility bills and verification of birth for all household members.
